BECARA, LLC PACT ACT INFORMATION FORM

In order to ensure compliance with the PACT Act, all current and future customers are required to submit or resubmit their business information. All accounts must fill out this form and input all information and numbers. We ask that any document that is required to run a business in your state also be submitted (Tobacco License, Business License, OTP License, Use Tax Permit, Sellers Permit). If any of the fields on this form are not applicable in your state, please enter N/A or not applicable.

If customers wish have multiple companies, this form should be completed for each company, and each address they will have orders shipped to. If applicable, documents for each location should be submitted as well.

All customers must enter their Federal EIN Number. We also ask that a copy of the EIN letter be submitted, if customer has on file.
